Hello and Welcome to Visioncall Opticians. We are Specialists in Nursing Home Eyecare.

Established in 2011, Visioncall Mobile Opticians embarked on a mission to provide exceptional eye care to the most vulnerable members of society. Over the years, we’ve grown and evolved, proudly becoming the market leader in mobile eyecare, now serving  Residents Of Nursing Homes in all 32 counties of Ireland.

Nursing Home Populations often have the greatest need for services when it comes to maintaining their health.

Visioncall Opticians are acutely aware of this need, which is why we regularly liaise with the management of all the nursing venues we provide eyecare to, working together to ensure that everyone has regular check ups to prevent and diagnose any potential conditions which may affect a person’s eyesight, and ultimately their quality of life.

 

Visioncall will always do our very best to maintain and improve  everyone’s Vision, so that we can all enjoy the richest experiences life has to offer. Each test is specific to the individual in front of us, putting them and their needs at the centre of what we do.

With clinical staff located in every region of Ireland, your visiting optician is always a local member of your community.

We’ll provide a detailed report for your care plans afterword. All glasses will be engraved, hand delivered & fitted.

How to use our service

Our Service is Free of Charge. We don’t ask any Nursing Home for any sign up or retention fees.

All Medical Card Holders are entitled to a FREE full eye exam and any glasses required every 2 years.

Eye Testing including screenings for many eye conditions. If a Person has been diagnosed with an Eye Condition such as Cataracts, Glaucoma, Age-Related Macular Degeneration or  they have Diabetes, then these people should have their eyes tested every 12 months. We’ll monitor who’s due, when and why. and liaise with you, working together to ensure everyone is up to date.

In order to have their application approved, a letter stating which condition that person has much be attached to their  HSE LOA3 Optical Benefit Application Form. Visioncall can provide these letters provided we’ve tested those people in the past.

It’s always best to contact us before submitting any requests for approval to the HSE, to mimimise letters of rejection.

How to Apply For Your Residents Eye test with their Medical Card

(This is the Official Procedure as per the HSE)

Step 1: Phone your local HSE office

Call Your Local HSE Office to Order LOA3 APPLICATION FORMS

Ask For The Adult Optical Department (Details Below).

Step 2: Request application Forms

Ask the HSE for Blue/Green “LOA’3” Application Forms (One for Each Resident), And White “Form B’S

(One… “Form B” Is Required for… Every 15 Blue/Green LOA’3 forms)

If 20 Residents are due ask for 20 forms… etc.

Step 3: Complete Paperwork

After requesting these forms, the HSE will post you the application forms.

*Please Note Each “LOA’3” Blue/Green Form requires TWO SIGNATURES for Approval

*Each White “Form B” Requires The Person In Charge (DON/ADON/CNM /SN) To Print Their Name In Capital Letters, Sign Their Name, and Give Their Title On The Upper Section**

These are the LOA3 forms.

They are individually numbered and cannot be photocopied.

 They are provided by the HSE direct to the Nursing Venue.

This is the Form B.

Please keep this form safe along with the LOA3 forms as it will be required by Visioncall at a later stage.

Please note that 1 Form B is required for any amount of LOA3 Applications not exceeding 15.

If there are more than 15 OA3 Applications a second Form B is required.

These Form Bs can be photocopied provided there is a FRONT & BACK.

Step 4: Post these forms to the HSE for Approval

Post Filled Forms To The HSE For Approval.

Approval may take 2-6 Weeks.

(If they are taking longer than this then It might be worth Phoning the HSE to Check Their Status.)

Step 5: Inform Visioncall Opticians once forms are approved

When Approved Forms Have Come Back in The Post let us know and We Will Then Arrange A Date To Come To Your Nursing Venue To Carry Out The Eye Examinations.

This step is very important as the forms are time sensitive.

We must conduct the Eye Tests within 90 DAYS or the whole process may need to begin again.

Conditions may include (but aren’t limited to):

 – A condition that affects your short term memory

 – Cognitive impairment

 – Dementia

 – Schizophrenia

 – Autism

 – Severe anxiety

 – A diagnosed mental illness

 – A condition that affects your mobility

 – Severe arthritis

 – An illness that requires constant medication (oxygen, for example)

 – A medical reason prohibiting you from leaving your home (vertigo, for example)

 – An illness that prevents a person from leaving the home unaccompanied

 – A medical professional has advised you not to leave home without help
